Death Certificates

Death Certificates are on file at the Town Clerk's office in the town where the death occurred, and where the person lived at the time of their death. Certified Copies can be obtained in person or by mail.

To recieve Certified Copies by mail, please print and complete the request form. Attach a copy of your driver's license or photo I.D. along with payment and send it to the Town Clerk's office. Requests are processed and mailed out the same day we recieve them.

Certified Copies are $20 each and payment is cash or check. Please make checks payable to Town of Avon.
